Whether your loved one is recovering from illness or injury, suffering from a chronic medical condition, or simply needs assistance with routine daily activities, CT Healthcare can customize a care plan that supports his or her medical and personal home care needs in the comfort and security of their own home.

A care manager serves as our clients’ personal advocate and is responsible for developing holistic care plans. When you contact CT Healthcare to discuss your loved one’s living situation, an experienced Healthcare  Administrator will conduct a screening over the phone to ensure that all of his or her needs and challenges have been addressed. CT Healthcare then offers a thorough in-home assessment during which the medical and social needs of the individual will be taken into consideration – as well as the family and caregiver’s wishes and concerns – in order to make the best recommendation for the individual.

Once a plan of care is established, a care manager oversees all medical and personal care and the client’s interdisciplinary team of caregivers which may include social workers, therapists, home health aides and nurses. The manager continues to conduct regular in-home assessments and also meets with the team to discuss the client’s progress and challenges. The plan of care is re-evaluated often to ensure the client is receiving the appropriate care and support.
